Sheeba obtained a BA in Biology & Society from Cornell University, where she could explore the intersection between science and social concerns, and the complex relationship between biological and sociocultural forces. From there she earned a Master of Health Services Administration degree from the University of Michigan, School of Public Health. Since then, she has committed her career to increasing access to healthcare, for those who have historically been marginalized and considered vulnerable. In the Medicaid space, she has managed and implemented a program for evidenced-based, person-centered policy design, managed primary care networks, designed and tested models of behavioral health integration, and launched innovative tele-mental health programs. She understands the impact of multiple funding streams, effects of psycho-social issues on health, and the strength of coalition building and stakeholder engagement.
Unite Us for Integrated Care for Kids (InCK) Model Grant Applicants
Helping InCK model grant applicants deliver integrated care for children and their families.
Are you looking for a simple solution to address children and their families’ multiple needs, securely share their information, and report on a wide range of services and outcomes?
In this webinar, Unite Us’ Director of Network Development, Sheeba Ibidunni, will walk through key strategies and best practices to support organizations to satisfy the requirements for the Integrated Care for Kids (InCK) Model Grant. Sheeba will cover critical questions around how to get started, convening community partners, and leveraging a best-in-class technology solution to ensure your community is set up for success.
Convening an entire community around a seamless system of care is not simple. However, with intuitive technology, you can strengthen your existing relationships with other organizations, facilitate bidirectional referrals across an array of service types, and support prevention, early identification, and treatment of your priority health concerns.
We can help. Check out our InCK checklist below to learn how.
